“Horizon Recovery Alerts” delivers HIPAA-compliant appointment reminders, scheduling updates, and two-way communication for treatment-related matters.

2. Opt-In Methods
You may enroll by any of the following:
• Website Contact Form – Check the SMS consent box on HorizonRecovery.com/contact.
• Intake Paperwork – Select “Yes” in the “Contact Preferences – SMS” section and sign the form.
• Text Command – Text START to 602-755-7858.
By opting in, you authorize Horizon Recovery (“we,” “our,” “us”) to send you recurring SMS messages.

3. Message Frequency
Message frequency varies based on your appointments and questions.

4. Cost
Message & data rates may apply. Carriers are not liable for delayed or undelivered messages.

5. Opt-Out
Text STOP to 602-755-7858 at any time to cancel. After you text STOP, we will send a single confirmation text and you will no longer receive messages.

7. Data Privacy
We use your mobile number solely for Horizon Recovery Alerts and do not sell, rent, or share your mobile information with third parties.
